What is the Transitions of Care Clinic?

The Transitions of Care Clinic is a follow-up appointment scheduled shortly after you leave the hospital—usually within 7–14 days. Appointments will be done virtually through MyOchsner. If an in-person visit is needed, our clinic is located on the third floor of the Comp Care Building which is to the left of the main hospital entrance at 1541 Kings Highway. Virtual appointments will be through your MyOchsner account.

During this visit, a healthcare provider will:

• Review your hospital stay and treatment

• Go over your medications

• Check your recovery progress

• Answer your questions

• Help prevent complications

What happens if you don't follow up?

Skipping your follow-up appointment can increase your risk of:

•Medication errors

•Complications

•Emergency room visits

•Hospital readmission
